This visit will also give you a chance not only to meet our doctors, office staff, and orthodontic technicians, but to see our office as well. Initial consultations also allow us to get to know you, your orthodontic concerns, and what your expectations are with treatment.

If you are choosing to complete the forms at our office, please arrive 10-15 minutes prior to your scheduled appointment time to ensure appropriate time.
